Attached you will find a Notice of Refused Clearance, property receipt and a voluntary abandonment form 3-2096. Please initial, sign and date the abandonment form in blocks 9 and 10 as indicated. Also sign the last space on the property receipt form 3-155 to verify receipt of a copy of this form, then email to ryanne_harris@fws.gov or fax the signed forms to (502) 247-2898. P.S. The strap was made from a protected species. It required a CITES permit to be imported. This document is acquired by the foreign entity, validated by that country's customs officers, declared to USFWS upon import and you must pay a $93 inspection fee (which you are still responsible for this shipment ).
